By completing registration, the Team Representative certifies that they have authority to bind all players, coaches, and team staff to the terms below, and that all parents/guardians of minor players have been informed of and consent to these terms.

PART 1 — ASSUMPTION OF RISK & RELEASE OF LIABILITY

I, on behalf of my team and all participating players and their families, acknowledge and agree that:

1. Participation in baseball and softball involves inherent risks including contact with pitched or batted balls, bats, other players, fences; running and sliding injuries; weather conditions; heat illness; and other hazards of outdoor athletic activity.
2. On behalf of all participating players and their parents/guardians, I voluntarily assume all risks of injury, illness, or property damage arising from participation in the AppleSmash Tournament.
3. I release, discharge, and covenant not to sue LYB, the City of La Crescent, field facility owners, tournament officials, umpires, and volunteers from any and all claims arising from participation in this tournament.

PART 2 — CONCUSSION & HEAD INJURY ACKNOWLEDGMENT

1. Any athlete suspected of sustaining a concussion will be immediately removed from play and may not return to play the same day.
2. An athlete removed from play due to suspected concussion may not return until cleared in writing by a licensed healthcare provider.
3. I have reviewed or will review the CDC HEADS UP Concussion Fact Sheet with all players and their families prior to the tournament.

PART 3 — MEDICAL AUTHORIZATION

In the event of a medical emergency, if a parent or guardian cannot be reached immediately, I authorize tournament medical staff or emergency services to provide emergency treatment as deemed necessary. LYB shall not be responsible for medical costs incurred.

PART 4 — PHOTO, VIDEO & MEDIA RELEASE

I grant La Crescent Youth Ball Association the right to photograph and video record players, coaches, and team activities during the AppleSmash Tournament for use in LYB publications, social media, website, and promotional materials.

PART 5 — TOURNAMENT RULES & CODE OF CONDUCT

1. All players, coaches, and spectators will abide by all tournament rules, LYB policies, and decisions of tournament officials and umpires.
2. Sportsmanlike conduct is required at all times. Ejection from a game may result in ejection from the tournament.
3. Tournament director decisions regarding scheduling, weather delays, and field conditions are final.
4. Entry fees are non-refundable except as provided in the weather/cancellation policy.

By completing registration and checking the acknowledgment box, the registering party certifies they are an authorized representative of the team, are 18 years of age or older, and agree to all terms above on behalf of the team, its players, and their families.
