2025 Season Registration

I, the undersigned parent/guardian of the above-named minor player, acknowledge that participation in youth baseball/softball involves inherent risks including but not limited to: physical injury from batted or thrown balls, sliding, collisions with other players or structures, and overexertion.

In consideration of my child being permitted to participate in La Crescent Youth Ball (LYB) programs for the 2025 season, I hereby release, waive, discharge, and covenant not to sue LYB, its officers, directors, volunteers, coaches, umpires, and agents from any and all liability, claims, demands, actions, and causes of action arising from participation in LYB activities.

I further agree to indemnify and hold harmless LYB and all persons associated with LYB from any loss, liability, damage, or costs incurred due to participation in LYB activities, whether caused by negligence or otherwise.

La Crescent Youth Ball may photograph or video record participants during games, practices, and events. These images may be used for promotional purposes including the LYB website, social media, print materials, and local media coverage.

No photos or videos will be sold, and player names will not be published with images on public social media without additional consent.

In the event of an emergency and I cannot be reached, I hereby give permission to the medical personnel selected by LYB coaches/volunteers to secure and administer emergency medical treatment for my child. I agree to pay all such costs.

A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. You cannot see a concussion and most concussions occur without loss of consciousness.

Signs and symptoms of concussion include: headaches or pressure in the head, nausea or vomiting, balance problems or dizziness, double or blurry vision, sensitivity to light or noise, feeling sluggish, foggy, or groggy, concentration or memory problems, confusion, or just not feeling right.

A child who is suspected of having a concussion must be removed from play immediately and may not return to practice or play until evaluated and given written clearance by a licensed health care provider trained in concussion management.

I acknowledge that I have read and understand the above information on concussions. I understand that if my child exhibits any signs or symptoms of a concussion, he/she will be removed from play immediately and may not return to play until cleared by a licensed health care provider. I consent to my child's coach removing him/her from play if a concussion is suspected.
