These waivers apply to all registered players in your family. Read each carefully and check to accept.

Liability Waiver

I, the undersigned parent/guardian, hereby release La Crescent Youth Ball (LYB), its officers, directors, volunteers, coaches, officials, and sponsors from any and all liability, claims, damages, actions, and causes of action arising from my child's participation in any LYB activity. I acknowledge that youth baseball and softball involve inherent risks of injury, and I voluntarily assume all such risks on behalf of my child. This release applies to injuries occurring while practicing, playing, or traveling to/from any LYB event.

Photo & Video Consent

LYB may photograph and/or video record participants during practices, games, and events for promotional and educational purposes including social media, website content, and printed materials.

Emergency Medical Authorization

I authorize LYB coaches and staff to consent to any emergency medical treatment for my child if I cannot be reached. I understand that LYB and its personnel are not liable for any costs incurred. I agree to provide accurate medical information and promptly update LYB with any changes.

Concussion Policy Acknowledgment

I have read and understand LYB's concussion policy. I understand the signs and symptoms of a concussion and agree that any player suspected of having a concussion will be removed from play immediately and may not return until cleared by a licensed healthcare provider. I agree to report any suspected head injuries to the coach or official immediately.

Registration Fee Policy

Registration fees are non-refundable after registration closes. The $$150 volunteer deposit is refundable upon completion of 4 or more volunteer hours during the season. The family registration maximum is $$350.00 — no family will be charged more than $$350 in player registration fees regardless of the number of players enrolled.

Request Financial Assistance — I would like to apply for LYB's scholarship program. No payment is required now — your roster spot is held while the LYB board reviews the application. The board secretary will email their decision. If a balance is still due after review, you can pay it from your Family Portal.

Volunteer Commitment

$$150 Refundable Family Deposit

LYB is entirely volunteer-run. A one-time $$150 deposit is included with your family registration — it covers all players. Complete 4+ volunteer hours during the season and we'll refund it in full.

Volunteer opportunities: field setup, concessions, scorekeeping, events, and more. Sign-up links sent after rosters are finalized.

Coach exemption: Families with a head or assistant coach on a team are exempt from the 4-hour volunteer obligation. Note: assisting at a single practice does not satisfy the volunteer requirement — exemption requires a full-season coaching commitment.
